Instructions - How to Score the PHQ-9

PHQ-9 scores can be used to plan and monitor treatment. This form will tally your total score for you when you click "Calculate Total Score" above. Interpret the score by using the guide listed below:

Score Action
0-4 Minimal Depression.Course skills may still be helpful.
5-14 Mild to Moderate Depression. Course skills may still be helpful. Continue to evaluate as you progress through this course.
15-19 Moderately Severe Depression. See a Doctor or Therapist for further evaluation. Course skills may still be helpful.
20 or higher Severe Depression. Treatment such as therapy and/or medication is required. Please seek immediate medical attention. Course may accompany ongoing therapy.
